Technical Assistance:
Discussed with the Director the medication authorization forms.

Comments:
An unannounced monitoring inspection was conducted today from 9:20am to 11:45am with the Director. There were 29 children in care from four months to four years of age being supervised by eight staff. The children were observed in center play, in free play with teachers, and during circle time. Five children?s files and five staff files were reviewed and in compliance. Coverage of CPR, First Aid, and Daily Health Observation trainings were verified. There were four medications on site and three Medication Administration Training (MAT) certified staff. The attendance, First Aid Kit, flashlight, weather band radio, and emergency drill log were reviewed and in compliance. The most recent Fire Inspection was conducted 9/17/19 and the most recent Health Inspection was conducted 1/4/19. Standard #: 22VAC40-185-510-A
Description: Based on review of four medications for three children, the center did not have written authorization from the parent to administer one medication to one child in care.

Evidence: For Child A there was a rescue breathing medication on site but there was no written authorization from the parent for the center to administer the medication.

Plan of Correction: We had a medication authorization form from the parent, but it was a shared form with the public school. The center staff will ensure that all medication forms are specific for their center and continue to maintain an accurate monitoring process on all documents.
