Comments:
A Subsidy Health and Safety Inspection was conducted on October 3, 2019 to review supplemental health and safety requirements. Licensing inspector arrived at 10:50am and departed at 1:40pm to conduct a Subsidy Health and Safety Inspection. There were 14 children present upon arrival to the center with five staff members. Additional staff arrived throughout the course of the inspection who were on site for administrative and support purposes. The children were observed during center time and playing freely in three classrooms. Five children's records were reviewed. Five staff records were reviewed. Medications are currently not being administered. The exit interview was conducted with the Program Director.

A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on observation and inspection of the facility, the vendor did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence: The center did not have a dated allergy list. The center director stated the allergy list was not dated.

Plan of Correction: The allergy lists have been dated.

Standard #: 22VAC40-665-510-B
Description: Based on observation and inspection of the facility, the vendor did not maintain a written hard copy record of daily attendance that documented the arrival and departure of each child in care as it occurred.

Evidence: In the infant classroom, there was no documentation of a written hard copy record of daily attendance that documented the arrival and departure of each child in care as it occurred on Thursday, October 3, 2019.

Plan of Correction: Director will double check attendance everyday.

Standard #: 22VAC40-665-520-B
Description: Based on record review, in four of five children's records reviewed, the vendor did not ensure that each child's record contained all required parent contact information.

Evidence: The work addresses of one or both parents were not documented in the records for Child #2, Child #3, Child #4 and Child #5.

Plan of Correction: The application to add work address.

Standard #: 22VAC40-665-520-B-11
Description: Based on on record review and staff interview, the vendor did not ensure a written allergy care plan for each child with diagnosed food allergy.
Evidence:
Child #1 had a diagnosed food allergy but there was not a written allergy plan from a physician.

Plan of Correction: Spoke to parent and should have the action plan by next week.
