Discussion was held on the topics of current enrollment, current staffing, and children files.

Comments:
The following inspection took place between the hours of 2 pm through 4 pm. Today, I met with the Staff of the County After-School Activities (CASA) program located on site at Forest Grove Elementary school in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 33 children observed with 2 staff; good ratio for this one group. A total of 5 children?s files were reviewed. Staff files were reviewed on an earlier basis. The children were engaged in the following: art, small motor skills, large motor skilled activities outside on the playground, bathroom breaks, hand washing, snack, home work time, and other organized activities. Standard #: 22VAC40-185-60-A
Description: Based on review, 1 of 5 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection, the Registration Page, which contains the basic information for Child #4, to include(but not limited to): the parent's work information, the complete information of two emergency contacts, and written agreement between parent and center was not available for review.
2. Staff #1 also conducted a search within digital storage and could not locate this information.

Plan of Correction: The main office shall be contacted, information shall be obtained, and placed on file. If information is found to be missing within main office, then parent shall be contacted and new information shall be completed.
