Comments:
A supplemental health and safety inspection was completed on September 6, 2019 from approximately 12:15 p.m. to 3:15 p.m. to review supplemental health and safety requirements. The inspection was initiated in response to specific compliance requirements the department was made aware of while investigating a self report regarding reports. An inspection was initiated on September 6, 2019, a collateral interview was conducted off site on September 3, 2019, a review of documentation was completed off site on September 9, 2019 and October 10, 2019, and the inspection was completed on September 6, 2019. Staff, children and classrooms were observed, staff were interviewed and children's records were reviewed pertinent to the incident. The director was available for the inspection and was present for the exit interview at which time the inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-550-2
Description: Based on a review of records and interview, the licensee failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence: 1. On September 3, 2019, Administrator #1 reported to licensing that Child #2 suffered a spontaneous nose bleed at the center on August 27, 2019, in which 911 was called and responded. On September 3, 2019, Administrator #1 reported to licensing that on August 27, 2019, the center was notified by Child #1's parent that medical treatment was obtained for an injury to the child that occurred at the center on August 26, 2019. 2. Administrator #1 acknowledged these injuries, in which a referral was made for medical treatment, were not reported within the required time frame.
