63.2 Facilities & Programs.
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, October 15, 2019 between 3:50 pm to 5:45 pm to determine the programs's compliance with licensing standards. The licensing inspector observed 39 children being supervised by 3 staff including the site supervisor. During this inspection, the children were observed by the licensing inspector completing a variety of to include transitioning indoor from outdoor play, completing bathroom breaks, eating snack, and participating in various center activities.The children's snack today consisted of baked plain chips, salsa, and water. The licensing inspector also completed observation of parent drop off and sign in during the inspection. The following information was reviewed: emergency supplies, required postings, fire and shelter-in-place drills, injury reports, and children's allergies.There were five children?s records and three of the staff's emergency contacts and required certificates of completion were reviewed during this inspection. Standard #: 22VAC40-185-60-A Description: Based on review of children's record and interview with staff, the center did not ensure all records contain the following information.
Evidence: In Child#4's record (start date 9/19) the name and phone number of child's physician was missing. Interview with staff confirmed the information was not available for review during the inspection.
Plan of Correction: Site director will continue to review files and follow up with parents to obtain missing information.
Standard #: 22VAC40-185-70-A Description: Based on review of staff information and interview with staff, the program did not ensure to maintain the name and address, emergency contact of all staff.
Evidence: The program did not have emergency contact information for Staff#3 during the inspection. Interview with staff confirmed the information for Staff#3 was not available for review at this time.
Plan of Correction: Site director will ensure that all staff information is kept in the licensing binder. Site director will ensure that all subs fill out a health info form before staff of shift.
Standard #: 22VAC40-185-540-A Description: Based on review of emergency supplies and interview with staff, the center did not maintained all required first aid items.
Evidence: The program's first aid kit was missing tweezers, and scissors during the inspection. Interview with the staff confirmed the required items were not available for review during the inspection.
Plan of Correction: Site director will complete the first aid checklist and make sure all supplies are stocked in the first aid kit.
Standard #: 22VAC40-185-540-E Description: Based on review of emergency supplies and interview with staff, the program did not maintain a working, battery-operated radio.
Evidence: During the inspection, the program was not able to provide a working, battery-operated radio.
Interview with staff confirmed the a working, battery-operated radio was not available.
Plan of Correction: Site director will ensure that a battery-operated radio is purchased and stored on site.
