A subsidy health and safety inspection was completed on October 10, 2019 from approximately 12:45 p.m. to 4:30 p.m. to review additional health and safety requirements. Ninety children were in care and approximately eighteen staff were on premises during the inspection. An additional thirty-eight children arrived from school at the end of the inspection with an additional three staff. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, outside play, transitioning to nap time, nap time, snack time, and arrival and departure from the center. Interviews and interactions were conducted and medications were reviewed. Nine children?s records and five staff records were reviewed. Three assistant directors were available for the inspection and one was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.
Standard #: 22VAC40-665-520-B-11 Description: Based on a review of records and interview, the licensee failed to maintain a written allergy care plan for a child with a diagnosed food allergy.
Evidence: 1. Child #5's record indicated he has a diagnosed food allergy. However, the record lacked a written allergy care plan as required. 2. In an interview with Administrator #3, she acknowledged the center does not have a written allergy care plan for the child.
Plan of Correction: After inspection, assistant director located the allergy plan in the front pocket of the binder. Said form has been hole punched and placed in the binder in alphabetical order along with the other authorization forms.
Standard #: 22VAC40-665-540-A Description: Based on a review of staff records, the licensee failed to have staff evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB), submitted at the time of employment and prior to coming into contact and completed within the last 30 calendar days of the date of employment for one of five staff.
Evidence: 1. Staff #3's record, hired on January 8, 2019, contained a tuberculosis screening/test dated January 10, 2019 and therefore not obtained within the required time frame. 2. Administrator #3 acknowledged that this tuberculosis screenings/test was not completed at the time of employment.
Plan of Correction: Assistant director has been counseled with regard to TB policy and will continue to be monitored by director for compliance.
