Comments:
An unannounced monitoring inspection was completed on 10/21/2019 from 9:50 A.M.-1:05 P.M. and from 2:00 P.M.-4:30 P.M. At the time of the inspection 44 children were in care with twelve staff, one office administrator, and one administrator present. An additional 14 children arrived after school and were in the care of two staff members. Seven children records, five staff records, six injury reports, emergency plan, first aid kits, evacuation maps, emergency supplies, health inspection, fire inspection, injury prevention plan, playground, and posted required information were reviewed. Children were observed during free choice activities, art activities with stampers, sensory activity with playdough, diapering, storytime, handwashing, lunchtime, transition to naptime, behavioral guidance, infant interactions, snack, arrival of afterschool children to the program, and afternoon center play. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of children records, the center failed to ensure that one out of six children had a physical on file within 30 days of attendance.

Evidence:
1. Child #4 did not have a physical on file. First date of attendance for Child #4 was 12/17/2018.
2. Staff verified that Child #4 did not have a physical on file.

Plan of Correction: Administration will contact the parent and obtain the physical to place in the child's file.

Standard #: 22VAC40-185-430-L
Description: Based on observation, the center failed to ensure that infant walkers shall not be used.

Evidence:
1. In Classroom #1 the Licensing Inspector observed three infant walkers in the classroom. Child #6 was observed to be sitting in a pink infant walker and pushing it on the carpet area.
2. Staff verified that there were three infant walkers in the classroom and that there was one walker in use.

Plan of Correction: Administration immediately removed the infant walkers from the classroom and placed them in the trash.
