Comments:
An unannounced monitoring inspection was conducted between the hours of 9:00 am and 12:00 pm. Upon arrival there were 53 children in care with 10 staff present. A sample of 5 children and 2 staff records were reviewed. Children were observed during circle time and playing in learning centers. Procedures for emergencies was discussed with the director.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a record review the center did not ensure all required information was kept for each staff person.
Evidence:
Staff #1 (hired 10/11/19) did not have documentation of having r written information to demonstrate she has the qualifications for her position as a program leader. The file also had staff #1 listed as a program leader on some paperwork and and an assistant on another.

Plan of Correction: The following corrective action was provided by the program director during the exit interview:
The director will obtain all required information for staff #1 and maintain it in the file.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
In the preschool room there was peeling paint in one of the children's bathroom stalls, in reach of children.

Plan of Correction: The following corrective action was provided by the program director during the exit interview:
The program director will contact maintenance and have the area of peeling paint repaired.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method.
Evidence:
In the school-age room there was an open box of Borax detergent on a shelf. The warning label read, "Keep out of reach of children" and "eye irritant."

Plan of Correction: The following corrective action was provided by the program director during the exit interview:
The borax was placed in a locked place during the inspection.
