Comments:
The licensing inspector conducted an unannounced monitoring inspection in response to a self-reported incident that happened at the center on 09/06/2019. Interviews were conducted. Two staff's records, policies and procedures were reviewed. Areas of non-compliance are identified on the violation notice. An Intensive Plan of Correction has been requested. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interviews, center staff failed to ensure care, protection, and guidance to a child they were supervising.
Evidence:
1. On 09/06/2019, at approximately 10:00 am, staff #1 and staff #2 left a 23 month old child (child #1) inside the center's toddler 2 classroom when staff and children left the classroom and building to go outside to the playground. Staff #1 stated, "That morning took kids to the playground. Skipped my mind to check board. (Attendance chart is posted on bulletin board that displays number of children in care) Child #1 don't talk, he may have hided [sic]. Maybe 25 - 30 minutes outside. I came back, child #1 smiled and I checked him. I usually count, but I don't know why I didn't check the board."
2. Staff #2 stated, "We had breakfast and then it is time to go outside. Staff #1 said lets go outside and said for me to take child #2. So I did and I had child #2 and another child with me. We go outside for 20 - 30 minutes. Staff #1 walked back into the classroom first. I could tell something was wrong and I saw child #1. He was on the couch, quiet and not crying."

Description: Based on interviews, center staff failed to have a child under 10 years of age always within actual sight and sound supervision.
Evidence:
1. Staff #1 and staff #2 reported that child #1 (a 23 month old) was left unsupervised inside the center's toddler 2 classroom on 09/06/2019. Staff #1, staff #2 and the children left the classroom and building at approximately 10:00 am to go outside to the playground. The staff and children were outside for approximately 20 - 30 minutes. Staff and children returned to the center classroom and child #1 was seen sitting on the couch. Child #1 was not within actual sight and sound supervision for approximately 20 - 30 minutes.
