Evidence:
1. On 09/06/2019, at approximately 10:00 am, staff #1 and staff #2 left a 23 month old child (child #1) inside the center's toddler 2 classroom when staff and children left the classroom and building to go outside to the playground. Staff #1 stated, "That morning took kids to the playground. Skipped my mind to check board. (Attendance chart is posted on bulletin board that displays number of children in care) Child #1 don't talk, he may have hided [sic]. Maybe 25 - 30 minutes outside. I came back, child #1 smiled and I checked him. I usually count, but I don't know why I didn't check the board."

We go outside for 20 - 30 minutes. Staff #1 walked back into the classroom first. I could tell something was wrong and I saw child #1. He was on the couch, quiet and not crying."
