Comments:
An unannounced inspection was conducted in response to a self-reported incident received in the licensing office on 10/16/19. Interviews were conducted with staff, records reviewed and observations of the program were made. A violation was cited in relation to the report and are documented in the violation notice.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on a self-report made by the center, interviews and a record review, the center did not ensure all children under the age of 10 years were in sight and sound supervision of staff.
Evidence:
On October 15th, 2019, a 3 year-old child #1 was discovered by a staff member, unsupervised, in a classroom for about 3 minutes.

At 4:52 pm child #1 was on the playground with 15 other children and 3 staff(#1, #2 and #3). At about 5:22 pm, Staff #3 went inside the building to clean-up her classroom before going home. At 5:25 pm Staff #3 passed through the preschool classroom to go back outside before leaving and saw child #1 inside the classroom, unsupervised. Staff #3 immediately brought child #1 to the office where he stayed with the Assistant Director. At 5:32 pm, Staff #1 realized child #1 was not in the group playing outside. Staff #1 and #2 brought the entire group of children inside,to look for child #1 to discover he was in the office.

Staff #1, #3 and #4 verbally confirmed that this was not the first time child #1 has tried to leave his classroom. Documentation in the child's record also confirmed he has tried to leave his classroom on other occasions.

Plan of Correction: The following plan of correction was provided by the acting program director during the exit interview:As a result of this incident, the as a result. The Acting Director went over playground safety and supervision with the staff. There will be follow-up training in the upcoming weeks.
