Comments:
An unannounced monitoring inspection was conducted today from 10:30am through 2:15pm. There were 62 children in care ages two months through four years old with 12 teaching staff and the program director. Children were engaged in teacher and self-directed activities including learning center play, lunch, nap, transitions, restroom routines and hand washing. Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation the center failed to maintain resilient surface as required.
Evidence:
1 - There is grass growing up through the resilient surfacing on the playground in and around the equipment and into the fall/use zones. Grass growing through the resilient surface negates the resiliency of the surface.
2 - Staff 1 confirmed that there is grass growing up through the resilient surface.

Plan of Correction: Grass was removed on 10/21/2019. Management reviewed with Safety Captain the playground checklist and Safety Captain will ensure resilient surfacing in within state regulations during daily checks.

Standard #: 22VAC40-185-440-E
Description: Based on observation and interview the center failed to ensure that occupied cots have at least 12 inches of between them.
Evidence:
1 - There are seven occupied cots in the toddler classroom that have approximately 1 - 4 inches of space between them.
2 - Staff 4 and 5 confirmed that the seven occupied cots have less than the required 12 inches of space between them.

Plan of Correction: This was removed on 10/21/2019. Management followed up with families about proper naptime materials. Management will inspect all bedding prior to the materials going into the classroom.

Description: Based on observation and interview the center failed to ensure that filled comforters are not used by children under two years of age.
Evidence:
1 - Child 1 in the older infant room was napping under a fiber filled comforter type blanket.
a. Child 1 is one year and two months old.
2 - Staff 2 confirmed that the child was sleeping under a fiber filled blanket.

Plan of Correction: Corrected on 10/21/2019. Management followed up with families about proper naptime materials. Management will inspect all bedding prior to the materials going into the classroom.

Standard #: 22VAC40-185-450-D
Description: Based on observation and interview the center failed to ensure that when pillows are used they are covered with pillow cases.
Evidence:
1 - In the 3 - 4 year old room child 2 is sleeping on a pillow without a pillow case.
2 - Staff 3 confirmed that child 2 was sleeping on a pillow without a pillow case.

Plan of Correction: Cot spacing was reviewed with staff. Staff will ensure and monitor proper cot spacing daily and continue to utilize 15 minute naptime check to assist staff with checking frequently. Staff and Management will create a naptime cot chart to include adequate spacing.
