An unannounced monitoring inspection was conducted today. There were 41 children in care, with a total of 3 staff assigned to their direct supervision. In addition, there were 40 children and 2 staff members attending an off-site SACC program field trip to play flag football at a nearby SACC location.
The children were observed eating lunch from home, reading books, enjoying circle time discussions, and selecting from various options for afternoon activities.

Standard #: 22VAC40-185-130-A Description: Based on a review of children's health records, it was determined that the center did not obtain documentation of a current immunization record before each child started to attend care.
The health record for child C and child D do not include documentation of a current immunization record. Each child entered care in October, 2019.
Plan of Correction: I will remind the school nurse that we need a copy of each shot record.
Standard #: 22VAC40-185-140-A Description: Based on a review of children's health records, it was determined that one record does not include documentation of a completed physical examination under the direction of a physician.
Child record E does not include documentation of a completed physical exam. This child has been in care for longer than 30 days.
Plan of Correction: I will ask the school nurse for a copy of the physical form.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that three records do not include all required information.
1 - Child record B does not include the father's work contact information.
2 - Child record C does not include the mother's work phone number.
3 - Child record E does not include the information for a second emergency contact person.
Plan of Correction: Each parent will be reminded to provide the information.
