Comments:
An unannounced monitoring inspection was completed on 11/04/2019. There were 17 children, ages 11 months-4 years, and 5 staff present during the inspection. The inspector reviewed 5 children's record and 5 staff records during the inspection. Staff stated that medication is not administered. Children were observed in the following activities: having breakfast, in free choice activities, during learning time and during arts and crafts. Proper hand washing, table sanitizing and diapering procedures were also observed. Infants were observed being fed and allowed to sleep on demand. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector arrived for the inspection at 8:15 a.m. and departed at 11:00 a.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-240-D-5
Description: Based on record review and discussion with the staff, the facility failed to ensure that there shall always be at least one staff member on duty who has obtained within the last 3 years instruction in performing the daily health observation of children.

Evidence: Staff #5 was the only staff member identified to have obtained within the last three years instruction in performing the daily health observation of children. Staff #5 left for the day at approximately 9:15 a.m. and at that point there was not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Plan of Correction: The Program Director stated the staff will complete an on-line course right away.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the facility failed to ensure that all electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence: There were 2 uncovered electrical outlets under the table beside of the changing table in the infant room; the outlets were at the ground level and accessible to children.

Plan of Correction: The outlets were covered during inspection.
