Comments:
The inspector conducted an unannounced mandated renewal inspection from 10:15 until 1:30. There were 28 infants and children in care. Infants were observed playing, eating lunch, and during rest time. Children were observed playing outside, a circle time, being read to, eating lunch, and during rest time. Five children's records and five staff records were reviewed. No medications were reported being administered. Please call your inspector at 276-698-9981 with any questions.

Description: Based on record review, the center did not receive all the required information for a child's record.
Evidence:
1. The inspector reviewed 5 children's records. Three of five records (#1,#2, #3) did not contain addresses for the emergency contacts.

Plan of Correction: Staff will get the info. from the parents.

Standard #: 22VAC40-185-330-B
Description: Based on measurement, the center did not have the required depth of resilient surfacing on the playground.
Evidence:
1. The inspector measured the mulch and it was 2-4 inches thick around the slides.

Plan of Correction: Staff will fluff and move mulch around and may add mats to maintain the required depth of mulch.
