Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 5, 2019. The on-site inspection was from 9:55 a.m. to 1:00 p.m. On this date, there were 12 children present in one classroom and in the care of two staff members. There were a total of 20 children enrolled in the morning preschool program. The violations from the previous inspection were reviewed for compliance. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were noted on the supplemental page. There were five children's records reviewed and the two staff members' records were reviewed. There were two staff members interviewed, as well as three children. The medication and medication paperwork were reviewed for one child. Snack was provided by the center and consisted of graham crackers, pretzels, and bananas. The children brought their lunches from home. The current unit was Fall and Scarecrows. The children enjoyed painting scarecrows, constructed scarecrows out of different shapes, and acting out the story, The Little Old Lady Who Was Not Afraid of Anything. Some items that were reviewed and observed included: art activities, calendar time, circle time, story time, hand washing and bathroom procedures, transitions, snack, outdoor play, lunch, dismissal procedures, warm water temperature, required posted information, injury reports, documentation of evacuation and shelter-in-place drills, emergency supplies, and first aid kit.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of two staff records and verification from the director, one staff member failed to obtain a negative tuberculosis (TB) screening within two years of the most recent screening.

Evidence:
1. The record for Staff #1 did not contain an updated TB screening within the last two years. The date of the most recent TB screening was 10/02/2017.
2. Staff #1 confirmed she had not obtained an updated TB screening.

Plan of Correction: Staff #1 will obtain an updated TB screening as soon as possible.

Standard #: 22VAC40-185-550-E
Description: Based on review of shelter-in-place practice drills and discussion with the director, there was no documentation of the most recent shelter-in-place practice drill.

Evidence:
1. There was no documentation that the staff had conducted a second shelter-in-place drill during the 2018-2019 school year.
2. The director reported that the second shelter-in-place drill for the year was conducted on 05/18/2019 but she had forgotten to document it.

Plan of Correction: The director documented the drill during the inspection and in the future, she will be sure to document all evacuation and shelter-in-place practice drills.
