An unannounced monitoring visit and facility tour was conducted from 10:15 am- 11:20 am. At the time of inspectors' arrival there were 61 preschool aged children in care with 11 staff members. Children were observed participating in indoor play, educational instruction and activities and transitioning between activities. A sample of 6 children's records and 6 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-270-A Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. In the boys restroom there was exposed rust on the partition walls.
2. In the girls restroom there was a broken switch plate on the light switch within the reach of children.Plan of Correction: Director will submit a maintenance request for repairs.
Standard #: 22VAC40-185-550-B Description: Based on record review an interview it was determined the center did not ensure that the emergency preparedness plan was reviewed and updated at least annually.
Evidence:
1. The emergency preparedness plan was last dated as updated on 8/13/18.
2. Staff #1 confirmed that the emergency preparedness plan dated 8/13/18 was the most current.Plan of Correction: Director will ensure the emergency preparedness plan is reviewed and updated annually.
Standard #: 22VAC40-185-550-E Description: Based on record review and interview it was determined the center did not maintain a record of the dates of the practice drills for one year.
Evidence:
1. The logs for evacuation and shelter-in-place drills were blank.
2. Staff #1 confirmed that they did not have the record of the drills.Plan of Correction: Director will ensure that all drills are logged and posted.
