An unannounced monitoring inspection was conducted today approximately 12:15 pm- 2:00 pm. There were 112 children supervised by 16 direct care staff. The physical plant, 5 staff records, 7 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The playground and surfacing was inspected. The children were observed transitioning to lunch and preparing for departure. Hand-washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Standard #: 22VAC40-185-560-G Description: Based on observation and staff interviews, food containers were not clearly dated.
1. 5 out 5 lunch boxes did not have a date on the box..
2. The provider was aware the lunch boxes had to be labeled to identify owner but did not know it had to be dated.
The food container shall be sealed and clearly dated and labeled in a way that identifies the owner
view of lunch boxes, not all lunch boxes are dated. Evidence: 3 of 3 lunch boxes were labeled to show ownership, but did not have a date on the box.
Plan of Correction: A system will be created and implemented to date all lunchboxes as they arrive daily.
