Technical Assistance:
Technical assistance was provided regarding reference checks and immunizations. When references are taken via telephone, documentation shall include dates of contact, name of person contacted, summary of reference, and signature of the person making the call. On the "Certificate of Religious Exemption" for immunizations, the form shall be signed by the parent and notarized.

Comments:
An unannounced renewal inspection was conducted today from 9:50am-11:55am. There were 2 children directly supervised by 1 staff. The physical plant, outdoor playground area, programming, menus, 1 staff record, 2 children?s records, 4 board officer files, evacuation drills, emergency supplies, and policies were inspected. There are currently no medications on site. Children were observed engaging in circle time, singing and dancing. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. A disinfectant spray with the label "keep out of reach of children" was observed on top of the paper towel dispenser in an unlocked bathroom and was accessible to the children in care.

Plan of Correction: It was moved and put in a cabinet with child safety lock.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review, required background checks were not repeated every 5 years as required.

Evidence:
1. Board Officer #2 (Start Date: November 2005) did not have the required Child Abuse and Neglect Central Registry background search completed every 5 years. Board Officer #2's most recent Child Abuse and Neglect Central Registry search was last completed on 10/28/2013.

Plan of Correction: We will get.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on record review, 4 of 4 board officer files did not contain the results of a fingerprint based national criminal history background check.

Evidence:
1. Board Officer #1 (Start Date: October 1996), Board Officer #2 (Start Date: November 2005), Board Officer #3 (Start Date: January 2004), and Board Officer #4 (Start Date: November 2016)'s records did not contain the results of a fingerprint based criminal history background check.

Plan of Correction: We will get.
