Comments:
An unannounced monitoring inspection was conducted on November 14, 2019. At the time of entrance there were 12 children and 2 staff members present with children. The sample size of records reviewed consisted of 5 children's records and 2 staff records. Children were observed reading books, preparing for rest period and during their rest period. There are no medications on site. The inspection began at 12:45 pm and ended at 2:15 pm. The results of the inspection were reviewed with the center director. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of 5 children's records and staff interview, the licensee did not ensue to obtain a copy of physical conducted before the child's first day of attendance or within one month after attendance that was conducted under the direction of a physician.

Evidence:
One record for child #2, did not include a copy of a physical. The first date of attendance was 9-10-2019.

Plan of Correction: Will get a copy of a physical within next two weeks.

Standard #: 22VAC40-185-150-B
Description: Based on a review of 5 children's records and staff interview, the licensee did not ensue to obtain documentation that each child has received the immunizations and each report shall include the date the immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.

Evidence:
One record for child #5, did not include a copy of immunizations received signed by a physician, his designee, or an official of a local health department. The first date of attendance was 9-30-2019.

Plan of Correction: Will get a copy of the immunizations within the next two weeks.

Standard #: 22VAC40-185-60-A
Description: Based on a review of 5 children's records and staff interview, the licensee did not ensue that each center maintained and kept at the center a separate record for each child enrolled with all required information.

Evidence:
Two children's records were determined to be incomplete as listed below.
1) The record for child #2 did not include the house number or apartment number for the home address for the first emergency contact listed and the second emergency contact listed did not include the street name and the house number or apartment number for their home address.
2) The record for child #1 did not include the house number or apartment number for the home address for the first emergency contact listed

Plan of Correction: Emergency contacts will be obtained with missing information.
