An unannounced complaint inspection was conducted on 10/25/2019 between the hours of 10:00am and 2:30pm and on 11/07/2019 between the hours of 12:00 pm to 4:30 pm in response to a complaint that was received by the licensing office on 10/16/2019 relating to operational responsibilities. Based upon a review of records, review of policies and procedures, staff interviews, and observation, the information gathered during this investigation does not support the allegations, so the complaint is determined to be not valid.
