Comments:
An unannounced focused inspection was completed on October 28, 2019 from approximately 11:15 a.m. to 12:15 p.m. to review compliance of specific standards. Sixteen children were in care and five staff were on premises during the inspection. Staff and children were observed in the classroom space, engaged in activities for infants. Interviews and interactions were conducted and six children?s records and one staff record was reviewed. The director and site supervisor were present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation, review of the license and interview, the licensee failed to ensure that the center's activities are maintained in compliance with the terms of the current license issued by the department. Evidence: 1. The center is licensed for children up to fifteen months of age . 2. In an interview with Administrator #2, she reported that one child currently enrolled and in attendance is seventeen months old.

Plan of Correction: The director has submitted a request to modify the license age range.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the licensee failed to post the most recent inspection of the facility.
Evidence: 1. Upon observation, the findings of the most recent inspection were not posted at the center. 2. Administrator #1 acknowledged these findings were not posted.

Plan of Correction: The director will post the most recent inspection.
