Comments:
An unannounced monitoring inspection was conducted today from 10:45am-3:15pm. There were 55 children (infants-4yrs.) directly supervised by 12 staff. The physical plant, 6 staff records, 6 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, playing with blocks, drinking milk, napping, being bottle fed, playing with legos, cutting patterns, playing outside and eating lunch. Today's lunch consisted of black beans, whole grain spaghetti, peaches, mixed fruit and milk. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center, were not maintained in a clean, safe and operable condition.

Evidence:
1. In the two's classroom, approximately 4" in circumference of peeling paint was observed on the back side of the entrance door. Chipped, peeling paint was also observed in several places along the window sill by the housekeeping area.
2. In the toddler classroom, chipped, peeling paint was observed on top of the half door and half wall that divides the toddler and the infant classrooms.
3.In the infant classroom, there are two sections of peeling wall board on the wall next to the classroom's storage closet. One area is approximately 4" long, the other area is approximately 6" long.
4.In the preschool classroom, a hole approximately 2" in circumference was observed under the children's cubbies exposing plaster and peeling paint, and chipping paint was observed in three different areas under the cubbies.
5. In the school-age classroom, two areas of approximately 4" of peeling paint were observed on the door that leads to the playground. The classroom's rugs had visible food particles and dust.

Plan of Correction: We will place emergency work orders today.

Standard #: 22VAC40-185-290-3
Description: Based on record review, not all unused outlets had child-resistant protective covers.

Evidence:
In the toddler classroom, two outlets were observed without protective covers.

Plan of Correction: We will replace the outlet covers.

Standard #: 22VAC40-185-510-D
Description: Based on review of medications, the center did not obtain a medication authorization for a long-term medication observed on-site.

Evidence:
Child#1 has 2 long-term emergency medications, there wasn't a medication authorization available for review during the inspection.

Plan of Correction: We have sent an email to the parents asking for it, we gave them a form but we have not received it back.

Standard #: 22VAC40-185-510-E
Description: Based on review of medication, the center did not ensure that a medication was labeled with the child's name.

Evidence:
Child#2 has a medication on site that was not labeled with the child's name.

Plan of Correction: Corrected during the inspection.
