Staff shadowing and training opportunities; open staffing positions.

Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, group time, story time, outdoor time, and personal hygiene were observed. Children were observed; and staff and children files and other documentation were reviewed. Thirty-nine (39) children, aged 3 months-5 years, were found to be in care with 7 staff and administrative staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:30 am and concluded at 10:50 am.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to immunizations.

Evidence:
Child record #1, enrollment date 8/9/19, did not have a physicians or designee signature and date on the immunization document as required.
Child record #2, enrollment date 10/16/19, did not have a physicians or designee signature and date on the immunization document as required.

Plan of Correction: The parents were contacted and asked to bring in a signed and dated immunization document. Once received the document will be placed in the record for compliance. All records will be reviewed by administrative staff for future compliance.

Standard #: 22VAC40-185-140-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to physicals.

Evidence:
Child record #1, enrollment date 8/9/19, did not have proof of physical as required and the allowed 30 days to obtain this had expired.
Child record #2, enrollment date 10/16/19, did not have proof of physical as required and the allowed 30 days to obtain this has expired.

Plan of Correction: The parents were contacted and asked to bring in a physical. Once received the document will be placed in the record for compliance. All records will be reviewed by administrative staff for future compliance.

Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to emergency contact information.

Evidence:
Child record #1, enrollment date 8/9/19, did not have emergency contact addresses as required.

Plan of Correction: This information will be obtained from the parents and recorded within the record to ensure compliance. All records will be reviewed by administrative staff to ensure future compliance.
