Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(8)-570 (Infant feeding); 22VAC40-185-(2)-60-A (Children's records); 22VAC40-185-(2)-70-A (Staff records)

Comments:
An unannounced monitoring inspection was conducted on 11/21/19 from 10:30am - 12:00pm. During the inspection there were 29 children ages six months old through four years old in care with six staff. Children were observed participating in various activities in the classrooms and eating lunch during the inspection. Records were reviewed for five children and five staff during the inspection. There was no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information.

Evidence
1. The record for child #3 did not indicate whether or not the child previously attended child care.
2. The record for child #4 did not indicate whether or not the child previously attended child care.
3. Staff #6 (Program Director) confirmed that the records for child #3 and child #4 did not contain all of the required information.

Plan of Correction: The facility responded: We will have the parents of child #3 and child #4 update the records.

Standard #: 22VAC40-185-350-E-1
Description: Based on on observation and interviews, it was determined that the licensee did not ensure that for children from birth to the age of 16 months, one staff member for every 4 children is maintained at all times children are in care.

Evidence:
1. There were two staff working in the Infant classroom with ten children. The facility was not maintaining the required 1 staff for every four children.
2. Staff #6 (Program Director) confirmed that the facility did not maintain the required staff-to-child ratio during the inspection.

Plan of Correction: The facility responded: The normal staff in the Infant classroom called out, another staff will be coming to assist the Infant Classroom. We will review with our the teachers the correct staff-to-child ratios and instruct them what to do when they have to many children.
