Comments:
An unannounced monitoring inspection was completed on November 7, 2019 from approximately 11:15 a.m. to 2:30 p.m. Fifty children were in care and thirteen staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include outside play, lunch, and nap time. Interviews and interactions were conducted and there are no children currently taking medications. Five children?s records and eight staff records were reviewed. The director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records, the licensee failed to ensure that a staff submit documentation of a negative tuberculosis screening no later than 21 days after employment or completed within 12 months prior to employment for three of eight staff.
Evidence: 1. Staff #4's record, hired on June 21, 2019, contained a tuberculosis test/screening dated October 7, 2019. Staff #7's record, hired on May 8, 2019, contained a tuberculosis test/screening dated August 21, 2019. Staff #8's record, hired on July 30, 2019, contained a tuberculosis test/screening dated October 14, 2019. Therefore none of these were obtained with the required time frame. 2. Administrator #1 acknowledged these tests/screenings were not completed within the required time frame of hire.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence: 1. Injury reports since May 2019 were reviewed. Approximately eleven of the reports were completed on forms that did not capture all the required information. This includes how the parent was notified, a second signature, future action to prevent injury, time parent notified, date parent notified, and staff present. 3. Administrator #1 acknowledged this information was not present on these injury reports. Administrator #1 reported there are two different forms being used by staff but that the center will use the correct form that captures all the required information for all injuries.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff records, the licensee failed to obtain a central registry finding within 30 days of employment for one of eight staff and employment was not discontinued.
Evidence: 1. Staff #2's record, hired on June 17, 2019, contained a central registry finding dated August 7, 2019. There was no documentation in the record of follow up with the office of background investigations when the results were not received within 30 days. 2. Administrator #1 acknowledged follow up to check on the findings has not been documented and that the results were not obtained within 30 days.
