Comments:
A unannounced monitoring inspection was conducted with the directors. There were 70 children in care with a age range of three months to five years old. children were observed during free play, rest time and Thanksgiving day dinner many family members were eating with their children. Eight staff and ten children's records were reviewed. Standard #: 22VAC40-185-260-B
Description: Based on review the annual health inspection was not current
Evidence: The date on the most current health inspection is 3/1/18.

Plan of Correction: We will call the health department and set up and appointment.

Standard #: 22VAC40-185-510-D
Description: Based on review of six children's medications not all had a parents authorization.
Evidence: Child A's medication authorization for one inhaler and a Auvi_Q expired 7/10/19.
Child B medication authorization for one Epi-Pen expired 6/30/19.

Plan of Correction: We will have the parents fill out ten day forms until the long term forms can be completed by the physician.
