Comments:
An unannounced monitoring inspection was conducted from 9:00 am-11:30. There were a total of 42 children with 7 staff members present. Children were observed during snack time, engaged in individual activities and celebrating a child's birthday. A selection of staff and children records, medications and other documents were reviewed.The playground and surfacing was inspected, there is a new play structure on the older children playground. Director stated that she did not renew the subsidy contract, no children are enrolled with subsidy. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, at least every two years from the date of the first initial screening the center has not obtained the results of a follow-up tuberculosis screening.
Evidence: Staff #1 most current TB screening results is dated 6/20/17.

Plan of Correction: We will have staff get TB within 30 days.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children records, the center does not have documentation of viewing proof of the child's identity and age.
Evidence: There is no documentation of viewing proof of identity and age for child # 1, date of attendance 8/26/19 and child #2, date of attendance 8/26/19.

Plan of Correction: We will document the proof of viewing birth certificate upon receipt.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the outside playground, the playground equipment does not have resilient surfacing that will comply with minimum safety standards.
Evidence: The playground for the smaller children was inspected, there are 3 small slides that are approximately 2 feet high. These slides do not have the required resilient surfacing to meet the minimum safety requirements.

Plan of Correction: We will remove the 3 climber slides.

Standard #: 22VAC40-185-550-D
Description: Based on a review of fire evacuation drills, the center has not implement a monthly practice evacuation drill and a a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.
Evidence: There is no documentation that a November fire evacuation drill was conducted and the last documented shelter drill is dated 3/29/17.

Plan of Correction: We will implement drills and document according to the guidelines.
