The licensing inspector completed an inspection in response to a self-reported concern regarding forbidden action that occurred at the center on 10/18/2019. The Department was notified of the incident on 10/18/2019. On 10/21/2019, the licensing inspector was on site from approximately 10:50AM until 12:20 PM interviewing staff, conducting observations at the center, and reviewing documentation. On 10/22/2019, the licensing inspector received information regarding the center's policies and procedures. Additional interviews were conducted on 10/28/2019. The preponderance of the evidence gathered during the investigation supports areas of noncompliance with the licensing standards related to forbidden action. See the violation notice for additional details.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measures.

Standard #: 22VAC40-185-400-B Description: Based on interviews and documentation review, the center failed to ensure behavioral
guidance was constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts when a staff pushed a child away to prevent the child from crawling on her.
Evidence:
1. Staff #3 stated ?[Witness #1] came to the office on Friday, 10/18/19 at approximately 5:30 PM and said she saw [Staff #1] push [Child A] down while looking into the Infant Room.?
2. Staff #2 stated ?[Staff #1] was sitting on the floor doing meal count sheets. Three babies were around [Staff #1], crawling on her and she yelled `I can?t get anything done? . [Staff #1] pushed [Child A] down. [Child A] fell down on some climbing mats and started to cry. I don?t think [Staff #1] was trying to hurt [Child A], I just think she was in a rush and trying to move [Child A].?Plan of Correction: Not available online. Contact Inspector for more information.
