A Subsidy Health and Safety inspection was conducted on December 3, 2019 to review supplemental health and safety requirements. Six staff were present with twelve children. Five staff and five children records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-500-F Description: Based on review, facility did not ensure there is a current written list of all children's allergies, sensitivities and dietary restrictions that is dated and kept confidential in each room or area where children are present.
Evidence: The record for child #1 had documentation of an allergy to berries and coconut. This information was not noted on the list in the classroom.Plan of Correction: Staff re-printed the list and added the information needed.
