Comments:
An unannounced renewal inspection was completed on 11/25/2019 from 9:30 A.M.-1:30 P.M. At the time of the inspection 52 children were in care with nine staff, one office assistant,and two administrators present. Five children records, six staff records,four board members background checks and reference letters, six injury reports, two medications and required documentation, health inspection, fire inspection, emergency plan, evacuations drills, first aid kits, emergency supplies, playground, injury prevention plan, playground plan, and posted required information were reviewed. Children were observed during handwashing, snack time, transitions, diapering, outdoor play time, small group activities that included journal writing, fine motor activities with beading, letter recognition activities,art activities, behavioral guidance, free choice activities that included building, coloring, dramatic play, lunchtime, and naptime. Exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on review of board members background checks, the center failed to obtain a sworn statement of affirmation within 30 days of taking office.

Evidence:
1. Board Member #2's file did not contain a sworn statement of affirmation.
2. Staff verified that the board member had held a position on the board for more than 30 days.

Plan of Correction: Administration immediately contacted the board member. The board member will come to the center by the end of the day to sign a sworn statement of affirmation, and it will be sent to the licensing inspector once completed.
