Comments:

An unannounced renewal inspection was conducted today. The inspector arrived at 11:00 AM and departed at 2:15 PM. Nineteen children were in care. Lunch service, nap time and afternoon snack were observed. A sample size of five staff records and five children's records were reviewed. Medication administration and injury records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #:	22VAC40-185-130-B
Description:	Based upon review of five children's records and staff interview, the facility has not ensured that updated immunization records are obtained once every six months for children under the age of two years. Evidence: 1. The most recent immunization record in the record provided for child 2 is dated 05/07/2019. 2. Staff 1 verified that the record for child 2 did not include updated immunizations since 05/07/2019.
Plan of Correction:	The facility responded with the following: We will ensure that updated immunization records are obtained every six months for children under the age of two years.
Standard #:	22VAC40-185-140-A
Description:	Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes documentation of a physical examination by a physician prior to enrollment of within thirty days of attending. Evidence: 1. The record provided for child 1, who enrolled on 9/3/2019, did not include documentation of a physical examination. 2. Staff 1 verified the enrollment date and that child 1's record did not include documentation of a physical examination.
Plan of Correction:	The facility responded with the following: The parent will be required to provide results of a physical examination. All children's records will be checked to ensure results of a physical examination are in each record.

Description:	Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes documentation of the child's first date of attendance. Evidence: 1. The record provided for child 1 and child 5 did not include documentation of each child's first date of attendance 2. Staff 1 verified that neither record had the first dates of attendance documented.
Plan of Correction:	The facility responded with the following: The first dates of attendance were added during the inspection. All children's records will be checked to ensure that the first dates of attendance are recorded.
Standard #:	22VAC40-185-70-A
Description:	Based upon review of five staff records and staff interview, the facility has not ensured that each staff record includes the date of hire and the name, address and telephone number for an emergency contact person, Evidence: 1. The records provided for staff 3 and staff 4 do not include the hire dates. 2. The record provided for staff 3 does not include the street address for the emergency contact person. 3. The record provided for staff 6 does not include the name and street address for the emergency contact person. 4. Staff 1 verified that information listed was not in the staff records as indicated above.
