Comments:
An unannounced monitoring inspection was conducted on Tuesday, November 19, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at 3:10pm and concluded the inspection at approximately 6:06pm. The center?s director was present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 21 children in the direct care of 3 staff members. Upon arrival, the children were engaged in a variety of activities. Each classroom was inspected and formal observations were completed. A group of children was observed doing homework and receiving assistance from staff as needed. Another group was observed taking bathroom breaks, washing hands, and preparing for snack. The children were later observed during transitions from snack, to homework, and free play. The center is equipped with toys and supplies and items are available to the children. Staff were engaged with the children and were observed offering guidance when needed. All areas where the children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. During the inspection, five children?s records and five staff records were reviewed. Medication is administered and three medications were reviewed today. The most recent evacuation drill was documented on 10/30/2019. The center is reminded to conduct two shelter-in-place drills by the end of 2019. The most recent lockdown drill was documented on 04/16/2019.

Standard #: 22VAC40-185-510-D
Description: Based on a review of three medications, medication authorizations, and interviews on 11/19/2019, the center did not ensure the medication authorizations for two children were available to staff during the entire time they were effective.

Evidence: 1) During the inspection, the licensing inspector observed a prescribed medication present at the center for Child #6. The authorization form did not have written authorization from the parent for facility staff to administer the medication.

3) During interview, a member of management confirmed the center did not have written authorization from Child #6 and Child #7's parents to administer the medications present at the center for each child.

Plan of Correction: Per the Center: The director will have the parents sign the authorization on 11/20/2019. In the future, the director will ensure all medication authorization forms are completed by all required parties.
