Comments:
A Subsidy Health and Safety inspection was conducted on November 25, 2019 to review supplemental health and safety requirements.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, three of five children's records reviewed did not contain all of the required information.
Evidence:
The records for Child 3, Child 4, and Child 5 did not contain the Name, address, and telephone number for each custodial parent's place of employment or school attendance.
