Comments:
An unannounced monitoring inspection was conducted on 12/3/19 from 11:30am until 2:10pm. At the time of the tour, there were 94 children in care with 12 staff and 1 administrator. A sample of 9 children's records and 9 staff records were reviewed. Children were observed playing outside on the playground, eating lunch and resting quietly during nap time. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of special care provisions and emergencies and background checks are identified on the violation notice and were discussed with the center administrator during the exit interview.

Violations:
Standard #: 22VAC40-185-540-C
Description: Based on observation and interview, the center did not ensure that first aid kit contains all the required items.

Evidence:
1. The center's first aid kid did not include triangular bandages.
2. The center administrator confirmed that the first aid kit was lacking triangular bandages.

Plan of Correction: Triangular bandages will be purchased and placed in the first aid kit.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain the results of a child abuse and neglect registry search from that state.

Evidence:
1. Staff 1 (date of hire: 9/3/19) indicated on her sworn statement or affirmation that she has resided in the states of Washington and New York within the past five years. Results of a child abuse and neglect registry search were not obtained from Washington or New York.
2. The center administrator confirmed that the results of a child abuse and neglect registry search were not obtained from the states of Washington or New York for staff 1.

Plan of Correction: A child abuse and neglect registry search will be requested from Washington and New York for staff 1. Going forward, sworn statements will be carefully reviewed to ensure that child abuse and neglect registry searches are obtained for any states in which staff have lived within the past five years.
