The licensing inspector conducted an annual staff record review on Wednesday, December 11, 2019 at the off site location. There were ten staff records for this site including substitute staff reviewed during this inspection. There were violations cited, see violations notice for details. Standard #: 22VAC40-185-160-C Description: Based on review of staff records and interview with staff, the program did not ensure all staff resubmit TB test results every 2 years.
Evidence: 1. In Staff#1's record (date of hire 09/08/18), the last dated TB was 10/18/16. 2. Interview with staff confirmed the TB results in the record was the most current during this inspection.
Plan of Correction: The employee is no longer employed. The Area Manager will audit all staff records by close of business on 12/20/2019 and obtain all missing documentation within 30 days to ensure compliance.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records and interview with staff, the program did not ensure to obtain a copy of results of a search of the central registry for all staff.
Evidence: In Staff#3's record (date of hire:8/19/19), there was no results of an out of state central registry found. Interview with staff confirmed documentation for an out of state central registry was not available for review during the inspection.
Plan of Correction: The Area Manager will audit all staff records by the close of business on 12/20/19. The employee will be on leave from the program until 01/06/20 and the signed central registry check will be obtained upon her return.
