Comments:
An unannounced monitoring visit and facility tour was conducted from 11:00 am-12:45 pm. At the time of inspectors' arrival there were 52 infant to preschool aged children in care with 10 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, lunch, indoor play, transitioning between activities, and toileting and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. The Director reported that no medications are given at the center at this time. Areas of noncompliance are detailed on the Violation Notice and were discussed with the Director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review and interview it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for child #5, under the age of 2 until May 2019 did not contain documentation of updated immunization documentation. The documentation was dated 1/11/19.
2. Staff confirmed that the most recent documentation of immunizations was dated 1/11/19.

Plan of Correction: Staff requested updated immunization documentation from the parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition.
Evidence:
There was a box of opened plastic gloves by the changing table in the 2 year old room. The gloves were at child height and presented potential choking hazards.

Plan of Correction: Gloves were moved during the inspection.

Standard #: 22VAC40-185-550-M
Description: Based on record review and interview it was determined the center did not ensure all written records of children's serious and minor injuries contained all of the required information.
Evidence:
1. The injury record for child #6 dated 9/19/19 did not contain the date, time, and method of parental notification as well as future action to prevent recurrence.
2.The injury record for child #1 dated 9/3/19 did not contain future action to prevent recurrence.
3. Staff confirmed that the injury reports were missing some of the required information.

Plan of Correction: Staff will add future action to prevent recurrence to the injury reports and remind staff to complete all components.
