Comments:
Thank you for your assistance during this mandated monitoring inspection. This on-site visit was conducted on 12/10/2019 from 9:30 A.M.-12:45 P.M. There were 24 children present with four staff and one administrator present. Five children records, six staff records, three injury reports, first aid and emergency supplies, evacuation drill, two emergency medications and required documentation, over the counter skin products and required documentation, health inspection, fire inspection, outdoor play area, and required posted information were reviewed. Children were observed during outdoor play time, transition to inside, handwashing, snacktime, group gathering, free play which included activities with building, reading, and puzzles, imaginative play in Santa's Workshop, indoor gross motor activity with dancing and throwing snowballs, behavioral guidance, arts and crafts, lunchtime, and half-day departure. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-60-A
Description: Based on review of children records, the center failed to ensure that all required information was in the child's file.

Evidence:
1. Child #4's file did not contain complete addresses for both emergency contacts.
2. Staff verified that the file did not contain complete addresses for both emergency contacts.

Plan of Correction: Administration will contact the parent and obtain the complete address for both emergency contacts. In the future upon enrollment administration will review all paperwork to ensure all required information is complete.

Standard #: 22VAC40-185-510-E
Description: Based on review of emergency medications, the center failed to ensure that all medication was labeled with the dosage amount and times to be given.

Evidence:
1. In Classroom #1 there was a bottle of emergency medication that was not labeled with the dosage amount and the times to be given.
2. Staff verified that the medication was not labeled with the dosage amount or times to be given.

Plan of Correction: Staff immediately labeled the medication with the dosage amount and times to be given.
