Technical Assistance:
We discussed the requirements for staff supervision. The risk assessment for a violation was provided.

Comments:
Thank you for your assistance during the unannounced monitoring inspection conducted from 2:55 PM to 5:50 PM. Today, there were twenty-one elementary school children attending the after school hours with two staff. Four volunteers were present for part of the program hours and assisted with children's activities. I viewed program activities, staffing, supervision, interactions with children, equipment, daily routines, emergency planning, emergency drills, injury records, agency inspections, snack, nutrition, five records for children, three staff records, staff qualifications, staff training, and indoor safety. The children enjoyed active play in the gym, snack, table activities and games in the cafeteria.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation of the activities and routines after check-in of children for the after school program, and interviews with a volunteer and staff, the program staff did not ensure that the children were always within actual sight and sound supervision of staff. Evidence: At the entrance corridor to the gym, by the restrooms, the licensing inspector talked briefly with staff member 5 as children entered the gym. Then, the Licensing inspector went into the gym and viewed children playing. When the licensing inspector no longer saw staff member 5 near the entrance of the gym, volunteer 4 was asked about the whereabouts of staff. During interviews with volunteer 4 and later with staff member 2 and 5, it was confirmed that staff member 5 was at the office, cafeteria and front hallway during dismissal and arranged to have office staff announce dismissal of the program children to the cafeteria. However, dismissal was already underway when the announcement was made. Staff member 5 was on the way to the school cafeteria after hearing the announcement to make sure children weren't waiting alone and was not aware that staff member 2 was located at this area of the school building. Staff member 2 approached staff member 5 in a hallway and they went to the gym. It was confirmed that from 3:46 PM to 3:50 PM there were twenty-one children, ages five through ten years, in the gym without staff supervision and were with four college student volunteers.

Plan of Correction: The director or assistant director will make sure the walkie-talkies are accessible and ready for use on all program days. The staff will carry and use walkie-talkies for communication so that all staff are where they are needed to supervise children when they check-in. The director will review supervision and staffing requirements with all staff, including substitute staff. The staff will make sure children are always within sight and sound supervision of children and will never rely on volunteers for supervision and staffing.
