Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe and operable condition.
Evidence:
The Toddler, Twos and Older Twos classroom had areas located on their window sills with chipped and peeling paint.

Plan of Correction: We will have the spots touched up by the end of the week.

Description: Based on observation and record review, the center did not ensure four infant records contained documentation of written parent authorization noting any known adverse reactions when diaper cream was used.
Evidence:
1. The record of child #4, child #5, child #6 and child #7 did not contain written parent authorization noting any adverse reactions for diaper cream.
2. Child #4, child #5, child #6 and child #7 had diaper cream labeled with their individual names.
3. The infant teacher stated that all diaper creams were used as needed within the past month.

Plan of Correction: We will send the new forms home to be signed. We will put a new system in place to ensure we have the correct form with the correct cream.
