Comments:
The licensing inspector conducted an unannounced monitoring inspection on 12/09/2019 from 1:39pm to 5:45pm. The children were observed learning through play in centers, singing, playing cames, washing hands and eating afternoon snack. A formal observation was completed with the School Age 2 and Preschool classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today as well as the facility?s mini buses.

Six children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Ten injury reports were reviewed.

The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure that two of six children's records had all of the required information.
Evidence:
1. The record of child #1 (start date: 9/30/19) listed one designated person to call in case of an emergency. All children's records require two emergency contact name's along with their address and phone number.
2. The record of child #5 (start date: 10/11/19) listed one designated person to call in case of an emergency. All children's records require two emergency contact name's along with their address and phone number.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe and operable condition.
Evidence:
1. The seats on van #1 were torn in several areas which exposed foam and pieces of torn vinyl.
2. The seats on van #2 were torn in several areas which exposed foam and pieces of torn vinyl.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The inspector observed an unlocked cabinet in the school age classroom which contained two cleaners.
2. The inspector observed unlocked chemicals on van #1, van #2 and van #3.
