The inspector conducted an unannounced mandated inspection from 2:30-4:30. There were 11 children in care with staff supervising at all times. Five child records were reviewed and four staff records were reviewed. Children were observed washing hands and having a snack, playing, watching a video, being picked up by their parents, and getting off buses to enter the center. No medications were reported being administered. Please contact your inspector at 276-698-9981 with any questions.

Evidence:
1. The inspector reviewed five staff records. One of five records (#3) did not have a repeat Tb test. The last one was completed 08/2019.Plan of Correction: Staff will get the repeat TB test/screening.
Standard #: 22VAC40-185-540-E Description: Based on observation, the center did not have a working, battery-operated radio.
1. The inspector observed a radio but there were no available batteries for it.Plan of Correction: The director will purchase more batteries.
