A unannounced monitoring inspection was conducted with one owner and the assistant director. 48 children were in care with an age range of three months to five years old. The children were observed during rest time, free play. Four staff records were reviewed and six children's records were also reviewed. Evidence: Child A with a start date of 5/20/2019 did not have the mother's work phone number.Plan of Correction: I will ask mother for missing information.
