An unannounced monitoring inspection was conducted today from 12:50pm-3:15pm. There were 76 children (infants-5yrs.) supervised by 18 direct care staff. The physical plant, 5 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe, and operable condition.

Evidence:
1. There was a broken child safety lock on a cabinet in the Infant A (children ages 6 weeks-16 months) classroom.
2. The mulched area of the ground on the large outdoor playground area has sunken areas around the playground equipment fallzones, creating tripping hazards.

Plan of Correction: We will put a new lock on the drawer. We have been putting in work orders since the beginning of the year for the sinking playground area and no one has come to fix it. We will put another one work order in today.

Description: Based on observation, the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Infant A (children ages 6 weeks-16 months), a cabinet low to the ground contained plastic sandwich bags and did not have a lock.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-320-B
Description: Based on observation, the center did not ensure that each restroom area provided for children was equipped with all required supplies.

Evidence: There was no toilet paper in the children's restroom in the 2's (children ages 2-3 years old) classroom.

Plan of Correction: We will put some in today.

Standard #: 22VAC40-185-440-E
Description: Based on observation, the center did not ensure that there was at least 12 inches of space between occupied cots, beds, and rest mats.

Evidence: In the 2's classroom (children ages 2-3 years old), there was less than 12 inches of space between cots that children were resting on.

Plan of Correction: We will look to see if we can limit the number of children in the 2's class. If we can not, we will restructure the class to make sure there is at least 12 inches between cots.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that disposable diapers are disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence: The foot-operated step trashcan was broken in the Infant A classroom (children ages 6 weeks-16 months).

Plan of Correction: I will give them the extra trashcan today.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center did not ensure that written record of children's injuries contained documentation of all required information.

Evidence:
1. 3 of 5 injury reports did not include documentation of the time of parent notification.
2. One injury report did not include documentation of the method of parent notification.

Plan of Correction: Corrected during inspection.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the center did not ensure that all applicable out-of-state central registry searches were requested.

Evidence: Staff B's (date of hire: 9/9/2019) record did not contain documentation that a central registry search for Washington state was requested.

Plan of Correction: I will submit it today.
