Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:30pm. There were 86 children (infants-5yrs.) supervised by 13 direct care staff. The physical plant, 4 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed coloring and cutting paper houses, engaged in a gross motor activity using indoor stepping stones, learning compound words, and eating lunch. Lunch served was chicken pasta, mixed vegetables, applesauce, and milk. Diapering and hand-washing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure that all children's records contained documentation of all required information.

Evidence: Child #3's (age: 22 months old) record did not contain documentation of proof of identity.

Plan of Correction: We will get missing information from parents.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Infant 1 classroom (children ages 6 weeks-16 months), a spray bottle containing bleach and water solution was sitting on the diaper changing counter in the corner of the sink area.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that a nonabsorbent surface for diapering or changing is used.

Evidence: In the 2's classroom (children ages 2-3 years old), there was a tear in the diaper changing pad.

Plan of Correction: We will order a new one today.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center did not ensure that written records of children's injuries contained documentation of all required information.

Evidence:
1. 3 of 5 injury reports did not contain documentation of the time of parent notification.
2. 2 of 5 injury reports did not contain documentation of the method of parent notification.
3. 1 of 5 injury reports did not contain documentation of staff and parent signatures or two staff signatures.

Plan of Correction: We will return them to the staff for completion.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not ensure that all staff had a central registry finding within 20 days of employment.

Evidence: Staff B's (date of hire: 10/4/2019) record contained documentation that a central registry request was mailed 10/4/2019. There was no documentation of central registry results or a 30-day follow-up.

Plan of Correction: We will follow-up with OBI.
