- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, December 17, 2019 from approximately 11:30 AM until 1:50 PM. A total of 16 children were present being supervised by three staff members.
The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed eating lunch, taking bathroom breaks and washing their hands, having the book ?The Gingerbread Man? read to them, looking at books and resting at naptime. Lunchtime consisted of pizza, greens, corn, fruit and milk. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: emergency supplies, daily attendance, practiced emergency drill logs, liability insurance policy, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Medication is administered, and medication and medication authorizations were reviewed. During the inspection, five children's and four staff records were reviewed.

Standard #: 22VAC40-185-40-J Description: Based on documentation review, the center did not ensure the injury prevention procedures were updated at least annually based on documentation of injuries and a review of the activities and services.
The injury prevention plan did not contain documentation as being reviewed. The injury prevention plan was the plan that was reviewed during the relocation modification that took place in November 2018.
Plan of Correction: We will review the plan and make any required changes if necessary. We will document that the plan has been reviewed.
Standard #: 22VAC40-185-70-A Description: Based on the review of four staff records, the center did not ensure three records contained the required information.
The records for Staff #2 (hire date 05/21/19), Staff #3 (hire date 06/05/19) and Staff #4 (hire date 05/06/19) did not contain the address for the emergency contact listed in the staff records.
Plan of Correction: I will have the staff add the address to the emergency contacts' name and phone number.
