Comments:
An unannounced monitoring inspection was conducted today from 9:00am-12:30pm. There were 52 children (infants-5yrs.) supervised by 10 direct care staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on observation, areas of the center are not being maintained in a clean, safe operable condition.
Evidence:
Peeling and chipped paint was observed in the 3's classroom, wall between the toddlers room and common area

Plan of Correction: I will contact maintenance to have them come out and fix the walls.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials are not being kept in a locked place using a safe locking method making them inaccessible to children.
Evidence:
The laundry closet was unlocked, leaving cleaning materials accessible to children.

Plan of Correction: Corrected during the inspection. We will make sure the door is always locked.

Standard #: 22VAC40-185-320-B
Description: Based on observation, each restroom is not equipped with toilet paper.
Evidence:
The bathroom in the 3's classroom does not have toilet paper.

Plan of Correction: Corrected during the inspection, we will make sure to check each bathroom each morning.

Standard #: 22VAC40-185-420-E-1
Description: Based on review of daily reports, at least 30 minutes of tummy time is not being recorded for infants who can not turn on their stomach independently.
Evidence:
There is no documentation of tummy time being recorded for any infants.

Plan of Correction: We will start recording it electronically today.

Description: Based on observation, cots are not being labeled for individual use.
Evidence:
Cots are not being labeled for individual use.

Plan of Correction: We will label the cots.
