Comments:
An unannounced Complaint Inspection was conducted on 12/2/2019 and amended on 4/1/20 in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations of lack of supervision. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegation, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 12/30/2019. Standard #: 22VAC40-185-40-E
Complaint related: Yes
Description: Based on staff interviews and review of the facility's policies around supervision of children, the facility failed to ensure that they followed the centers own policies and procedures that are required by these standards.
Evidence: Reviewed the facility's policies and procedures and found that staff did not follow supervision policies and procedures. Staff 4 and 5 left a child on a school bus, unsupervised for approximately 15 minutes, and were unaware that the child was missing due to not following the policies on supervision and field trips. Staff 4 did not complete a walk-through of the bus ensuring it was empty, and Staff 5 did not verify that the bus was empty before parking the van. Neither staff ensured that all of the children were accounted for upon arrival to the facility by comparing written lists, head counts and roll calls and; therefore, did not know that they left one child on the bus.

Plan of Correction: There was a staff meeting with the staff involved to discuss this incident that same day. The following day re reviewed the CCLC filed trip policy along with the sight and sound policy. Since then we also have put a sign in the back of the bus/van that staff have to apply and remove often checking for occupancy and remove when occupied. The field trip policy will be refreshed every beginning of summer and the new year, besides at orientation.

Description: Based on staff interviews, the facility failed to ensure that children under 10 years of age are always within actual sight and sound supervision of staff.
Evidence: Identified in staff interviews that Child 1 was left on the school's bus sleeping, for approximately 15 minutes, while the child's teachers, Staff 4 and 5 were on the playground with the rest of the students. Another staff member noticed the child on the bus and brought the upset child into the school where they were comforted until the parent's arrival. Staff 4 and 5 were unaware of the child being left behind on the bus until being notified by office staff.

Plan of Correction: There was a staff meeting with the staff involved to discuss this incident that same day, reviewing the sight and sound policy. We met the next day to discuss sight and sound as well as accounting procedures, roll call, enrollment, headcounts, written lists, etc.

Complaint related: Yes
Description: Based on staff interviews, the facility failed to ensure that staff verify that all children have been removed from the vehicle at the conclusion of any trip.
Evidence: Identified through staff that Staff 4 and 5 did not verify that all children were removed from the vehicle at the nd of the field trip, resulting in Child 1 being left unsupervised in the vehicle for approximately 15 minutes.

Plan of Correction: Review of the field trip and supervision of sight and sound policies. All staff signed these policies and a copy placed in their personnel files. Specific info on step by step accounting procedures was covered.
