Comments:
An unannounced monitoring inspection was conducted today from 9:00am to 12:00pm with the Director and Assistant Director. There were 72 children in care, ranging from five months to nine years of age, supervised by 12 staff. The children were observed finishing breakfast, having free play, playing on the playground, practicing writing, and putting together puzzles. Diaper changing was also observed in the Toddler room. Nine childrens records and five staff records were reviewed. The Center has staff with current certification in CPR and First Aid as well as Daily Health Observation. Two medications and authorization forms for one child were reviewed and were in compliance and the center has 10 staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The most recent documented evacuation drill was dated 12/11/19 and the most recent documented shelter-in-place drill was dated 6/6/19. The most recent Fire Inspection on file was dated 8/1/19 and the most recent Health Inspection on file was dated 1/24/19. Breakfast today was yogurt, strawberries, and milk. Standard #: 22VAC40-185-130-A
Description: Based on review of nine childrens records, the center did not obtain documentation that each child had received immunizations required by the State Board of Health before the child attended the center. Evidence: Child B, with a start date of 8/1/19, did not have documentation of immunizations in their record.

Plan of Correction: We emailed the families to request these during the inspection.

Standard #: 22VAC40-185-140-A
Description: Based on review of nine childrens records, each child did not have documentation of a physical examination by a physician on record within one month of attendance. Evidence: Child A, with a start date of 11/18/19, and Child B, with a start date of 8/1/19, did not have documentation of a physical in their records.

Plan of Correction: We emailed the families to request these during the inspection.

Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records, the center did not ensure that staff obtained and submitted the results of a follow-up Tuberculosis (TB) test or screening at least every two years from the date of the initial test or screening. Evidence: The most recent documentation of a TB test on record for Staff A was dated 9/1/17.

Plan of Correction: The staff will get this done right away.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center was not maintained in a safe condition. Evidence: On the playground, four out of five bicycles had cracked and broken plastic seats that posed a scratching and pinching hazard.

Plan of Correction: We removed the bicycles during the inspection.
