Comments:
An unannounced complaint investigation and a monitoring inspection were conducted today from 11:45am-3:00pm in response to a complaint received by the Fairfax Licensing office related to daily activities and programs. Based on staff interviews and classroom observations, the preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There were 109 children (infants-5yrs.) directly supervised by 32 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed being bottle fed, having tummy time, participating in group play, listening to stories, playing outside, and napping. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Violations not related to the complaint were observed during the inspection and are identified on the Violation Notice.

Description: Based on record review, the center did not obtain documentation of a physical examination by or under the direction of a physician within one month after attendance.

Evidence:
Child#3's record did not contain a physical examination, the child's first day of attendance was documented as 7/16/2019.

Plan of Correction: The parent of Child#3 was notified on 12/04/2019 and submitted the updated form on 1/2/2020.

Description: Based on observation, the center did not maintained areas of the center in a safe and operable condition.

Evidence:
Three ceiling tiles were observed with approximately 2 feet in circumference of stains from water damage in the restroom located in the library room.

Plan of Correction: All tiles were replaced on 12/05/2019.

Description: Based on observation, the center did not keep hazardous substances in a locked place using a safe locking method that prevents access by children.

Evidence:
Bleach bottles, detergent, and antibacterial foam containers were observed on an upper shelf in the center's laundry room, the door to the room was closed but unlocked.

Plan of Correction: Although all containers were out of the reach of children, a lock was installed on the laundry room door.

Description: Based on review of medication, the center did not follow procedures to prevent use of outdated medication.

Evidence:
The nonprescription medication observed on-site for Child A expired on 5/2019. The nonprescription medication observed on-site for Child B expired on 9/2019. The medications were not returned to the parents or disposed of by the center to prevent use of outdated medication.

Plan of Correction: The parents of Child A and Child B were contacted and new medication was provided on 12/05/2019.

Evidence:
In the Butterflies room, a food container was observed in the refrigerator labeled with the child's name but without a date.

Description: Based on observation, the center did not ensure that all infant bottle were dated.

Evidence:
In the Butterflies room, two infant bottles were observed stored in the refrigerator labeled with the child's name but without a date.

Plan of Correction: The missing date was corrected by the parent during the inspection.
